Case Scenario 3: End-of-Life Decision-Making

Kamala is an 80-year-old woman with end-stage heart failure post a myocardial infarction. She has been receiving acute care but has now been referred to your palliative team as she is approaching the terminal phase. She is unable to communicate her wishes clearly due to her deteriorating health. Unfortunately, Kamala has not had access to early palliative care and has not made an advanced care directive.

Kamala’s adult children Raj, Priya, and Amrita are divided over whether to continue life-sustaining treatment or transition to comfort-focused care. Raj, her oldest son, insists on pursuing all available medical interventions and hopes for recovery, including cardiopulmonary resuscitation and transfer to intensive care. Raj has power of attorney. While this allows for financial decisions on Kamala’s behalf, it does not stipulate medical or lifestyle decisions. Raj argues he has had this conversation previously and states his mother wants all treatment done. Priya and Amrita believe their mother would prefer a peaceful and dignified death and argue that they do not believe their mother would request such a thing.
